Submit a claim

If you forget your card on the day of treatment, you’ll have to pay the full amount charged by your healthcare provider but you’ll be able to claim by yourself. Please be aware that you should submit the claim within 2 years after the date of service.

Quick claiming with App

If you can’t use your card to claim on the spot, keep your receipt and submit a claim through the App. Simply take a photo of your receipt (make sure it’s not blurry and that all details can be read!) and submit. Easy!

Prepare claim document：a photo of your official provider receipt which include the following information:

  • the Provider’s name, provider number and address;
  • the Patient’s full name and address;
  • the date of service;
  • the item number/numbers where applicable and description of the service;
  • the amount(s) charged; and
  • any other information that nib may reasonably request.
